Does the application for a TABC license or permit relate to a business that operates or plans to operate as a sexually oriented business on the premises?

Is this a new or existing business?

This is a new business: (1) that will sell alcohol for consumption on-site or is applying for a mixed beverage permit, private club registration permit, or wine and beer retailer's permit, and (2) will operate a sexually oriented business on the premises.

This is an existing business: (1) that operates under a TABC mixed beverage permit, private club registration permit, or wine and beer retailer's permit or sells alcohol for consumption on-site; and (2) currently operates a sexually oriented business on the premises (if you are a member of the public filing a protest of this type of application, your protest can only be granted if it is supported by a petition signed by at least 50% of the residents within 300 ft. of the applicant's business; a copy of the signed petition can be attached to this form to satisfy this component).

I request an administrative contested case hearing based upon the above information. I understand that if my request is granted, I may be named as a party to a legal proceeding in which I bear the burden of proof regarding my grounds for protest. I understand that my protest is public information and that the commission may forward it to the applicant in its entirety, including my name, address and grounds for protest.
